Breast Reduction or Reduction Mammoplasty is a procedure that reduces the size of large, sagging breasts and creates smaller more youthful appearing breasts. Women frequently choose this operation because of back, neck and shoulder pain caused by the large size of their breasts. The great physical discomfort that is caused by very heavy, large breasts can limit participation in many activities or exercises, In addition, breasts that are out of proportion to a women's body can cause embarrassment and extreme self-consciousness.
Breast reduction can be performed at any age on a healthy woman, but it is usually recommended to wait until the breasts are fully developed. Some women decide to wait to have the surgery until their childbearing is over.

The Procedure
As with the Breast Lift surgery there are several different techniques and incisions that can be used to reduce the breasts. The most common procedure involves an anchor-shaped incision that circles the areola, extends downward, and follows the natural curve of the crease beneath the breast. The surgeon removes excess glandular tissue, fat and skin, and moves the nipple and areola into their new higher position.
Other variations of the incisions are also available. The technique used depends on the size of the breast. These include an incision around the nipple areola and a vertical scar, or a “J” shaped scar.
Liposuction may be used to remove excess fat from the lateral chest, or tail of the breast. Such a procedure will define and shape the breast.
The newer techniques in breast surgery are designed to minimize the amount of scars, while preserving a youthful shape to the breast.
In most cases the scar can be either limited to around the nipple-areola complex, or for larger breasts a vertical scar going down from the areola to the breast crease. With this technique the incision under the breast is eliminated. We have been using this technique with great satisfaction.
In some situations liposuction alone can be used to reduce the size of the breast. This method will leave minimal scar if any. However the skin envelope is not reduced and left to shrink around the new breast volume. Liposuction for reduction mammoplasty is ideal for the young patient with tight non stretched skin of the breast.
Breast Reduction by Liposuction
Massive breast enlargement can be a source of embarrassment and self consciousness about body image.
Many women with large breasts complain of upper back pain, shoulder pain, neck pain, and even arm numbness. These patients will benefit from breast reduction surgery. However, breast reduction surgical techniques leave significant scarring over the lower half of the breast. This significant scarring can be a source of increased self consciousness of this important cosmetic part of the body.
In these women most of the breast is composed of fatty tissue. Therefore liposuction becomes a viable alternative to the classical breast reduction surgery. Breast reduction by liposuction leaves very small scar of about 1/8 of an inch on each breast.
The scar is well hidden. It would be logical to assume that breast reduction by the liposuction technique would be more frequently done and requested.
This is a very well described technique, but the downside is that it is successful in a very limited patient population.
The ideal candidates for this technique are women that have good skin tone and fatty breast tissue. Although youthful breasts are firmer and more glandular they still contain large amount of fatty tissue. As women age, the glandular tissue is replaced with fatty tissue, but the older age and breast feeding stretches the skin and the skin loses its elasticity. In these cases the liposuction of the breast will lead to a droopy and sagging breast.
Ideal Candidates for breast reduction by liposuction:
- Women with large breast and firm skin
- Women who do not have excessively large sagging breasts
- Women who do not mind having a smaller sagging breast to relieve the pain and discomfort
Ultrasound assisted liposuction is utilized in young women with dense breast tissue to help extract the most amount of fat possible. Ultrasound liposuction may also allow some additional skin tightening from the heat energy generated from the ultrasound energy.
